Intermittent poor throttle response? Possible egr?

Jumped in my disco this morn and reversed out of my space and when I put it into D and pressed the throttle it only went to just above tick over even if I pushed pedal to the floor. I put it into N and revved it a few times which it did fine, put it back into D and it drove fine then, but later on today I came up to a sudden steep uphill and the car didn't accelerate or change down just let the revs gradually drop to about 1200 and crawled up (wasn't going that fast anyway) and I pushed the pedal right to the floor which made no difference, when I pulled off from the junction at the top of the hill then I booted it and got slow acceleration till about 2k then it took off like a rocket with a small puff of black smoke, did this once or twice then drives fine again! Are they connected or have I got two faults here?
Thanks!

Glad I found this forum before I bought one because I wanted an 07 till I read about faults/tax band/egr valves and decided on an early 06 instead! Bow down

07MY was a big change in the D3's life, and most of the common faults were rectified. However, they took a step backwards with the oil pump casing issues...
