A few things, check if its been egr'd blanked and that the butterfly valve has been removed, check the Turbo actuator rod is not seized up, clean the Maff sensor and also get a remap on the PCM.

I had a similar issue pulling onto roundabouts and junctions and it was completely resolved when I cleaned the MAP sensor. 5 minute job and costs nothing so worth a try.
Have a read of the below thread (if you haven't tried cleaning your MAP sensor already).

With it being a D4 you need to check the TSOV valve is not stuck, its on the drivers side and is just behind the radiator, its a common thing on the D4. The linkage seize up, its driven by a vacuum pipe from a chamber at the front of the engine.
You need to remove the big pipe and valve to check its working.
