I was concerned about a cracked manifold.....perhaps the smaller leaks were so obvious that it might need another smoke test to confirm there are no further issues.

Usually when the wife is driving....so under hard acceleration/load

Have had exactly same on mine. Never from cold, decent run-turn off then restart and get restricted performance message(no actual loss of performance). Fault read on cheapo code reader and no faults. Fault read with IID tool and got water in fuel sensor circuit fault
Changed fuel filter and not had issue in 600 miles (touch wood) 8)Previous Discos

'Modern' engines still have valve overlap don't they to scavenge exhaust gases?

I had a similar issue with our “old” MY2010 car. It only did it when towing the caravan and accelerating. If I stopped, closed the car and left it for 30 seconds it always cleared, but it was heart in mouth time every time we put the caravan on. It turned out to be something on the secondary turbo (actuator? - it’s a few years ago now!), which made sense as I got the message and lack of performance just about at the point where the secondary turbo would be kicking in (2,800 - 3,000 rpm ISTR).
