Injector leaks - what actually is the cause?

It seems quite a few of us (and on other platforms) have had the leaky injector syndrome and had them replaced. But does anybody know what causes the leak in the first place; component failure or carbon build up etc...

both, component failure (wear due to poor diesel, poor filter, ......) and/or carbon build up that bring the injector to leak
