Information on repeated clutch failures please

I've done a few searches and revealled a range of issues relating to manual D3s... I am on a quest to find out as much as possible about the possible reasons for a D3 requiring multiple clutch replacements.

So far I have found:

Replaced clutch
Replaced clutch & flywheel
Replaced release mechanism (thrust bearing?) & clutch.

Seems to be a handful of members who have had problems ranging from just 11,000 mile up to around 40,000
Does anyone have experieince of replacing several clutches in a short space of time or distance covered and what was eventually found to be the problem?

Any help would be appreciated and I'll see if I can get a mention for the site in a LR enthusiasts publication as this is where the info is going to end up.

Iona , 06 my and replaced clutch & flywheel at 100,000 klms.Was a known issue with some 06 my's and there are threads on it here somewhere.

Edit to Add : This TSB was applicable in my case and due to clutch slip.

TSB 308-001
A customer may report a concern of one or more of the following symptoms:
This bulletin has been re-issued to include a closing VIN.
Strong odors from under vehicle when, or after, a slow speed maneuver (e.g. difficult
parking maneuver), especially when on gradients.
Clutch slip.
The clutch pedal has stuck in the downward position or not releasing fully.
Difficulty in engaging gears.
Cause: The clutch drive plate lining material is not tolerant to high heat inputs caused by
slipping, becomes soft and is then damaged beyond recovery even when it has cooled.
Action: Should a customer express concern regarding the above, refer to the Service.Procedure detailed in this bulletin to renew the clutch assembly.

My Nov 05 had its clutch and flywheel replaced quite early in its life, around 16k.

Al

Now gone - MY16 D4 SE Tech, Loire Blue, Almond Leather, Privacy, plus some other goodies.
Old - MY12 D4 SDV6 XS Auto - Ipanema Sand with Almond Leather - Plus other niceties, and D4.com sticker
Older - D3 TDV6 XS Auto - Lugano Teal with Almond Leather, 20" Stormers, Shiny Tailpipes, DVD/TV - and obligatory D3 sticker
Ancient - D3 TDV6 S - Tonga with Ebony, 20" Stormers, satnav & DVD

2010 Discovery 4 3.0 TDV6 HSE Manual Zermatt SilverDiscovery 4

New clutch at 30k -replaced with the upgraded version. Repeat failure at 32K -had to be very assertive to get clutch + flywheel replaced under warrenty. I wonder whether it was the flywheel at fault in the first place
